D.O.T. PHYSICALS

Under federal law, operators of commercial motor vehicles are required to undergo a physical examination at least once every two years. The courteous staff at our clinic offers these exams in a comfortable, professional environment.

Your DOT physical includes:

  • Urinalysis
  • Vision exam
  • Hearing exam
  • Blood pressure test
  • Heart rate test
  • Ear/nose/throat exam
  • Reflex test
  • And additional physical examinations

Each physical is performed by a doctor certified by the National Registry of Certified Medical Examiners.

 

Prior to your physical appointment please complete the following:

  1. Make sure you drink plenty of water as you will need to provide a urine specimen for testing.
  2. Please bring your driver’s license and most recent DOT/medical card with you if you have been issued on in the past.
  3. Please click on the link provided below to send us a secure email of all of your medications and dosages and the reasons for taking each medication such as the diagnosis or health problem that they are prescribed for. This list is to include any and all prescription drugs, vitamins, and OTC medication you are taking.
  4. If you have ever had heart issues or heart disease (including but not limited to heart attack/myocardial infarction, artery bypass, stent, ablation, atrial fibrillation, arrythmia, murmur, tachycardia, aortic aneurysm, preventricular contractions or angina pectoris) please download the cardiovascular assessment form at the link below and have your cardiologist fill it out prior to your appointment.
  5. If you have any issues with diabetes, please email securely at the link below copies of your most recent A1c readings and blood sugar numbers for the previous calendar year. The examining doctor for your DOT physical may require you to get a letter of clearance from your treating physician depending on your medical history and lab values presented.  Please email lab values securely at the link below as soon as possible for review.  If you are insulin dependent, please click on the following link to download the Form MCSA-5870 and make an appointment with your physician that is treating you for your diabetes so they can fill out the form no more than 45 days prior to your DOT Physical in our office.   6. If you have been diagnosed with obstructive sleep apnea you are required to use a C-Pap machine and a printout of the previous calendar years usage is required to show compliance. You may securely email that to the link below.
